Food or food business complaint form

If you would like to register a complaint regarding food purchased from a food business located within the City, or on a food safety issue, the following form can be completed.

Where possible, a sample of the food should be kept by wrapping it in plastic wrap and placing it in a freezer, so that it can be tested later if required. Any evidence such as receipts of purchase should also be kept. Should you suspect that you have suffered illness from food, it is recommended that speak to your doctor and request relevant laboratory tests to identify a potential food poisoning.
